HAP’s network coverage areas

With our fully insured HMO plans, you can seek in-network care in the Michigan counties listed below*. Counties not listed below are considered out-of-network:

*Seven counties in northwest Ohio are also considered in-network in your HMO plan: Defiance, Fulton, Henry, Lucas, Ottawa, Williams and Wood.

With our fully insured PPO plans, your employer must be located within one of the counties listed below, but you are able to seek in-network care in any county in Michigan and northwest Ohio*. You can also use the Aetna Administrators® Network When outside of the HAP Large Group PPO network:

With our fully insured EPO plans, your employer must be located within one of the counties listed below, but you are able to seek in-network care in any county in Michigan and northwest Ohio*:
